School Holiday Program
2019 Winter Holiday Enrolment
Please fill in the form below for all participants for School holiday programs.
If you are an existing Altitude Member we just require this form to be submitted.
If you are not a member of Altitude Gym Sports - please ensure that you have filled in the 2019 Registration Form prior filling in this School Holiday Program, as we will require your contact and other important details, such as medical info.
BOOKING FEES & DETAILS
Bookings must be received by 3.00pm Friday of the week prior to holidays.
Confirmation and Invoices for payments will be sent once minimum numbers are reached.
Please book early so that we can ensure that all sessions / days will proceed.
Morning Single Days - $25 members; OR $30 non-members
Half Day Single Days - $40 members; OR $45 non-members
Full Day Single Days - $55 members; OR $60 non-members
Full week (5 days)
Morning Full Week - $100 members; OR $125 non-members
Half Day Full Week - $175 members; OR $200 non-members
Full Day Full Week - $250 members; OR $275 non-members
SCHOOL HOLIDAY PROGRAM - General Attendance
Age and Ability restrictions - Children attending general holiday program must be 6 yrs to 14 yrs.
Children must be at least 6 years of age, especially if non-members. However, if a child is attending Altitude and is 5 years old, please ask management if the holiday program would be suitable for that child. Please note that the suitability of each child under 6 years will be considered individually, case-by-case.
Children who are over 14 yrs who wish to attend are also welcome, especially if attending with siblings or a group of friends.
LACK OF INTEREST – CLASSES RISK CANCELLATION
If there is a lack of interest for the particular session, particularly the full day sessions, they will be cancelled. To avoid the disappointment of sessions being cancelled, please make sure that all bookings are made early.
