Altitude Gym Sports Pty Ltd (ACN 149 982 552) (“AGS”)
As legal guardian I hereby consent to the person(s) participating in a Gymnastic session (Program). Programs include, but are not limited to - Classes, Trial Classes, School holidays, Parties, and Gym-Hire. I recognize that potentially severe injuries can occur in any activity involving height or motion, including but not limited to, tumbling and related activities including Trampoline, Mini-tramp, Acrobatics, Gymnastic Activities, Rope climbing, Trapeze, Bungee & Harness activities and physical activity in general. I understand that it is the express intent of all staff and personnel to provide for the safety and protection of the child and, in consideration for allowing the child to use these facilities. I hereby COVENNANT NOT TO SUE and FOREVER RELEASE AGS, the gym centre, and all owners, lessors, staff, coaches, contractors, sub-contractors, teachers, and volunteers, involved in these programs, from all liability and for any and all damages and injuries suffered during instruction, supervision and/or control during any and all Gymnastic sessions. By participating in a Program with AGS I accept the gym centre’s rules, policies and this waiver.
